Counseling Services Student Referral Form

If you are currently in crisis and need to be evaluated immediately by a mental health professional, please contact 911 immediately. This form should be emailed to counselingNY@nyfa.edu. Students will be contacted directly by Jacky Hunt, LCSW on the next day she is in the office. Students will be evaluated and scheduled for sessions after their initial intake session with the therapist.
